13 Cariës en chloorhexidine

Samenvatting

Chloorhexidine kan met succes worden toegepast voor reductie van plaque en gingivitis, ter ondersteuning van wondgenezing, bij bestrijding van mondinfecties en ter voorkoming van tandcariës. Chloorhexidine is verkrijgbaar in preparaten voor thuisgebruik zoals spoelvloeistof, spray, gel en tandpasta en in preparaten die professioneel moeten worden aangebracht zoals chloorhexidinegel en de chloorhexidinevernissen (tabel 13.1). De principiële vraag kan worden gesteld of de preparaten voor thuisgebruik wel geadviseerd moeten worden in de kindertandheelkunde. Immers, de problemen waarvoor chloorhexidine wordt ingezet, kunnen en moeten opgelost worden door verbetering van de mondhygiëne. Dit is het primaire doel van de preventie in de kindertandheelkunde. Vervanging of aanvulling hiervan door chloorhexidinepreparaten kan de aandacht te veel van dit primaire doel afleiden. Bovendien zijn er tal van praktische problemen die doen twijfelen aan de bruikbaarheid van chloorhexidinepreparaten voor thuisgebruik in de kindertandheelkunde. De bestaande chloorhexidinetandpasta's bevatten geen fluoride. Er bestaan wel spoelmiddelen met 0,12% chloorhexidine en 0,05% NaF. Het is echter nog onbekend hoe effectief de fluoride in een dergelijk preparaat is. Bovendien kan spoelen het poetsen niet vervangen. Chloorhexidine moet dus altijd worden gebruikt in aanvulling op de normale mondhygiëne. Dikwijls wordt het advies gegeven dat er een uur tijd moet zitten tussen het gebruik van chloorhexidine en fluoridetandpasta en dat voor het poetsen met chloorhexidinegel of -tandpasta een andere tandenborstel gebruikt moet worden dan voor het poetsen met de gewone tandpasta. Het nut van deze adviezen is overigens nooit aangetoond. Daarnaast kent het gebruik van chloorhexidine een aantal bijwerkingen (tabel 13.2) die ongunstig zijn voor de therapietrouw aan chloorhexidineadviezen. Het middel smaakt vies. Er kan irritatie van gingiva en mondslijmvlies optreden, die varieert van roodheid en zwelling tot desquamatie. Ook de smaakgewaarwording kan door chloorhexidinegebruik verminderen. De meest frequente bijwerking is de mogelijk (geel)bruine verkleuring van elementen, tong, slijmvliezen, vullingen en vullingranden.
