Voeding bij epilepsie

Abstract

Dit hoofdstuk gaat in op de pathofysiologie, etiologie, klinische verschijnselen en de behandeling van epilepsie. Epilepsie en de voorgeschreven medicatie kunnen invloed hebben op de eetlust en het gewicht van de patiënt. Zowel overgewicht als ondergewicht zijn vaak voorkomende problemen. De meeste epilepsiepatiënten kunnen met anti-epileptica afdoende worden behandeld. Bij 25 procent van de patiënten heeft medicatie echter geen of onvoldoende resultaat. Dan moet een andere behandelmethode overwogen worden, bijvoorbeeld chirurgie of een ketogeen dieet. Een ketogeen dieet wordt gekenmerkt door een hoog percentage aan vet (70-90 energie%), een laag percentage aan koolhydraten en een normale hoeveelheid eiwit. Tot nu toe werd het dieet vooral bij kinderen gebruikt, maar inmiddels wordt het ook bij volwassenen toegepast. Er zijn verschillende vormen van het ketogeen dieet: het klassieke ketogeen dieet, het MCT-dieet, het gemodificeerd Atkinsdieet en het dieet met laagglykemische index. Er zijn meerdere hypotheses die het werkingsmechanisme kunnen verklaren.
