Voeding bij de zuigeling en het jonge kind met een aangeboren hartafwijking

Abstract

Bij kinderen met een aangeboren hartafwijking komen voedingsproblemen en ondervoeding als gevolg van de ziekte regelmatig voor en kunnen het genezingsproces en de ontwikkeling van het kind negatief beïnvloeden. Een groot deel van deze hartafwijkingen wordt chirurgisch gecorrigeerd. Palliatieve chirurgie in het geval van een complexe hartafwijking heeft als doel de circulatie te stabiliseren zonder deze te kunnen normaliseren. Het zijn vooral deze patiënten die langdurige voedingsproblemen hebben en ernstige groeiachterstand kunnen ontwikkelen. Ondervoeding komt het meest voor bij zuigelingen en jonge kinderen tot de leeftijd van 3 jaar. De inhaalgroei na een chirurgische correctie van hartafwijkingen met belangrijke hemodynamische consequenties wordt daarnaast negatief beïnvloed door intra-uteriene en genetische factoren en een laag geboortegewicht. Voedingstherapie dient vroeg gestart te worden om preoperatieve groeivertraging te voorkomen.
