Ticstoornissen bij kinderen: welke behandelingen zijn bewezen effectief?

Samenvatting

In dit artikel geven wij een overzicht van de wetenschappelijke evidentie voor werkzaamheid van bij ticstoornissen beschikbare behandelvormen. Voorafgaand aan de behandeling moet goede diagnostiek verricht worden, ook van eventuele comorbiditeit. Daarnaast is het van belang de ernst en hinder die het kind van de tics ondervindt en het wisselende beloop van de tics in kaart te brengen. De Yale Global Tic Severity Scale is hierbij een goed hulpmiddel. Het doel van de behandeling is de hinder die de tics veroorzaken, te verminderen. Als de tics niet hinderlijk zijn, kan volstaan worden met psycho-educatie.

De effectiviteit van zowel medicatie als gedragstherapie is aangetoond. Wat betreft de medicamenteuze therapie zijn op basis van effectiviteit en bijwerkingenprofiel pimozide en risperidon de middelen van eerste keuze. Haloperidol kan aangemerkt worden als tweede keuze. De werkzaamheid van clonidine is niet eenduidig. Gezien de milde bijwerkingen en het positieve effect op ADHDkenmerken en slaapproblemen, is het voorschrijven van clonidine in sommige gevallen verdedigbaar. Ook een aantal andere geneesmiddelen is in al dan niet placebogecontroleerde studies effectief gebleken. Deze middelen zouden alleen voorgeschreven moeten worden in gespecialiseerde centra.
