Samenvatting

Chronische aandoeningen zoals cardiovasculaire aandoeningen vormen de grootste operationele en financiële uitdaging in het gezondheidszorgsysteem in de komende 10 jaar. Ongeveer 1 op de 100 in de totale bevolking en 1 op de 10 in de populatie ouderen hebben hartfalen (chronisch) waarbij herhaalde ziekenhuisopnames voor HF niet ongewoon zijn.2 Onderzoek onder steekproeven van patiënten met HF rapporteren heropnamecijfers rond 47 procent in drie maanden en 54 procent in 6 maanden3- 4 hetgeen een betekenisvolle aanslag op de gezondheidszorgvoorzieningen met zich meebrengt.
