Modern management van chronische ziekten in de eerste lijn: e-zorg ten voeten uit

Samenvatting

Veelvoorkomende chronische ziekten in de huisartsenpraktijk – zoals diabetes – zijn in theorie goed te managen, mits aan een aantal voorwaarden is voldaan. Optimaal zelfmanagement en compliance van de patiënt hebben een belangrijke voorspellende waarde voor het behandelresultaat.
