Samenvatting

In een Cochrane-review concludeerden Gøtzsche en Olsen onlangs dat borstkankerscreening geen overlevingsvoordeel biedt. Hun manier van meta-analyse van de negen beschikbare screeningstrials bevat echter sterk subjectieve elementen. Buiten meta-analyse blijven de screeningstrials met positieve resultaten. Hun argumenten daarvoor blijken niet steekhoudend, behalve voor de Edinburgh-trial. De stelling dat borstkanker als uitkomstmaat onbruikbaar is, is niet gefundeerd. Anders dan beweerd zijn er geen aanwijzingen voor vertekening in de trial- uitkomsten in het voordeel van screening. De mening dat screening geen overlevingsvoordeel biedt, afgemeten aan de algemene sterfte, is onjuist. De Cochrane-review biedt geen wetenschappelijke grond voor twijfel aan de effectiviteit van borstkankerscreening voor vrouwen boven de 50 jaar.
