16. Dunne darm

Samenvatting

De dunne darm strekt zich uit van de pylorus tot aan het caecum en bestaat histologisch uit drie entiteiten: het duodenum, het jejunum en het ileum. De indicaties voor chirurgie aan de dunne darm zijn zeer divers en kunnen variëren van complicaties door adhesies, ischemie, (traumatische) perforatie, obstructie (paralytische of mechanische ileus), Meckel-divertikel, bestralingsenteritis en morbus Crohn. Daarnaast kan het gaan om een dunnedarmtumor. Daarvan vormen neuro-endocriene neoplasieën (NEN) de grootste groep (44 %), gevolgd door het adenocarcinoom (33 %), lymfomen (15 %) en sarcomen, waarvan de gastro-intestinale stromatumor (GIST) het bekendst is. In dit hoofdstuk zal vooral de pathologie en resectie van het ileum en jejunum besproken worden.
