Samenvatting

Lage gezondheidsvaardigheden vormen een belangrijke barrière in zelfmanagement van diabetespatiënten. Systematische interventieontwikkeling is nodig om de ondersteuning van deze patiënten in de huisartsenpraktijk zodanig aan te passen dat ook deze grote groep patiënten de mogelijkheid krijgt tot adequaat zelfmanagement. In dit artikel beschrijven we de resultaten van een probleemanalyse onder patiënten en zorgverleners. De resultaten van deze probleemanalyse worden gebruikt voor de ontwikkeling van een interventie voor zorgverleners.
