Samenvatting

Anafylaxie is een algemene, in principe levensbedreigende allergische reactie die snel na contact met een allergeen (insect, voeding, geneesmiddel) kan ontstaan. Soms wordt er geen oorzaak gevonden voor de anafylactische reactie en moet de patiënt als noodmedicatie een antihistaminicum en een adrenaline auto-injector voorgeschreven krijgen. De behandeling van een ernstige reactie bestaat uit het zo snel mogelijk toedienen van adrenaline, gevolgd door het geven van antihistaminica en corticosteroïden.

Bij insectenallergie bestaat er een absolute indicatie voor immuuntherapie bij een ernstige anafylaxie en een bevestigde IgE-gemedieerde reactie voor dat insect (huidtest, serologie). De immuuntherapie is veilig en de onderhoudsfase kan worden uitgevoerd in de huisartsenpraktijk, mits uitgevoerd volgens protocol en in overleg met de specialist. Bij deze kuur moet na de injectie altijd minimaal 30 minuten worden gewacht. Immuuntherapie is effectief voor bij, wesp en hommel en wordt over het algemeen vijf jaar volgehouden.
