Persönlichkeitstypen und Persönlichkeitsstörungen bei stationär behandelten Alkoholabhängigen
Abstract
Zusammenfassung.Theoretischer Hintergrund: In der Literatur werden bestimmte Persönlichkeitseigenschaften oder Persönlichkeitstypen mit der Entstehung und Aufrechterhaltung des Alkoholismus in Beziehung gebracht. Fragestellung: Es soll ein Beitrag zur Beantwortung der Frage geleistet werden, in welchen Persönlichkeitseigenschaften und Persönlichkeitsstörungen sich Alkoholabhängige von Kontrollpersonen aus der allgemeinen Bevölkerung unterscheiden und ob das Konzept einer begrenzten Anzahl von Alkoholabhängigentypen fruchtbar ist. Methode: In einer Querschnittstudie wurden 95 männliche und 56 weibliche stationär behandelte Alkoholabhängige mit den 42 Skalen des Trierer Integrierten Persönlichkeitsinventars (TIPI-Version 1) und dem Strukturierten Klinischen Interview zur Erfassung von Persönlichkeitsstörungen (SKID-II) untersucht und mit einer Kontrollgruppe von 199 Personen aus der allgemeinen Bevölkerung verglichen. Ergebnisse: Eine logistische Regressionsanalyse mit Kreuzvalidierung zeigte, dass 14 von 42 TIPI-Skalen eine gute Prädiktion der Gruppenzugehörigkeit (Alkoholabhängige vs. Kontrollpersonen) ermöglichen. Auf clusteranalytischem Weg konnten vier Persönlichkeitstypen identifiziert werden, die bei Alkoholabhängigen signifikant häufiger auftreten als in der normalen Bevölkerung. Bei 41% der Patienten wurden Persönlichkeitsstörungen diagnostiziert. Schlussfolgerung: Die Kenntnis von vier Typen von Alkoholabhängigen, die sich in ihrer Persönlichkeit und in Persönlichkeitsstörungen unterscheiden, kann zur ätiologischen Hypothesenbildung und zur Therapieplanung beitragen.
Abstract.Theoretical background: In many publications, certain personality traits and personality types are discussed in the context of the etiology and maintenance of alcoholism. Objective: The study aims at answering three main questions: Are there any significant differences in personality traits and personality disorders of alcoholics and a control group out of the general population? Is the concept of a limited number of types of alcoholics fruitful? How many alcoholic in-patients obtain a diagnosis of personality disorder? Method: In a cross-sectional study, 95 male and 56 female alcoholic in-patients were investigated using the 42 scales of the Trier Integrated Personality Inventory (TIPI-version 1) and the Structured Clinical Interview for Personality Disorders (SCID-II). Test results were compared with those of a control group of 199 subjects from the general population. Results: A logistic regression analysis followed by a cross-validation indicated that 14 of the 42 TIPI scales can predict group membership, i.e., alcoholics vs. control group. By means of cluster analysis, four personality types could be identified that are significantly more often found among alcoholics than among the general population. 41% of the in-patients obtained a diagnosis of a personality disorder. Conclusion: The differentiation between four types of alcoholics characterized by different personalities and personality disorders can contribute to the formulation of etiological hypotheses and to more adequate approaches in therapy.
