Abstract

Hartfalen (decompensatio cordis) is een klinisch syndroom dat gekenmerkt wordt door het tekortschieten van de pompwerking van het hart, leidend tot een complex van klachten en verschijnselen (tabel 1.2). Om de tekortschietende werking van het hart op te vangen treden compensatiemechanismen in werking, zoals bijvoorbeeld activatie van het renine-angiotensine-aldosteronsysteem.3 In de loop der tijd zijn veel definities voorgesteld (tabel 1.3),4 maar door het ontbreken van een gouden standaard voor het vaststellen van de aan- of afwezigheid van hartfalen heeft geen van deze definities wijde navolging gekregen. Voor het graderen van de ernst van hartfalen wordt vaak de classificatie van de New York Heart Association (NYHA-classificatie) gebruikt (tabel 1.4).
