Fibromyalgie

Samenvatting

Fibromyalgie is een lichamelijk onverklaard pijn-moeheidssyndroom met aspecifieke klachten: naast moeheid die het dagelijks leven hindert en pijn in spieren, spieraanhechtingen en in en om gewrichten, heeft meer dan de helft van de patiënten last van stijfheid, slaapproblemen, concentratieproblemen en/of buikklachten. Fibromyalgie gaat vaak samen met stress, depressie of angst en HPA-as ontregelingen lijken een rol te spelen in de pathogenese. Het spontane beloop is langdurig. Cognitieve gedragstherapie gecombineerd met oefentherapie vormt de beste behandeling en leidt er meestal toe dat de patiënt weer min of meer normaal kan functioneren. Een goede arts-patiëntrelatie, waarin de klachten en opvattingen van de patiënt serieus worden genomen, en een duidelijke uitleg van in stand houdende factoren zijn noodzakelijk om patiënten tot therapie te motiveren.
