Samenvatting

Zorg uit Voorzorg is gericht op het verminderen of voorkomen van beperkingen in activiteiten. Het is een van de interventies die in het kader van het Nationaal Programma Ouderenzorg werden uitgevoerd en met elkaar de stappen identificatie/selectie, assessment, opstellen plan, uitvoeren plan en monitoren gemeen hebben. Zorg uit Voorzorg wordt in dit hoofdstuk als exemplarisch voor comprehensive geriatric assessment interventies beschreven.
