De kernvraag bij dementie is niet waarom de één wel en de ander niet dement wordt, maar waarom de één het vroeg wordt, en de ander laat.
In het preklinisch stadium kan, in het individuele geval, geen enkel pathologisch kenmerk van de ziekte van Alzheimer met zekerheid de ontwikkeling van dementie voorspellen.
De traditionele grenzen tussen de verschillende vormen van dementie, zoals tussen de ziekte van Alzheimer en vasculaire dementie, en tussen Lewy-bodydementie (DLB) en parkinsondementie, vervagen steeds meer.
Door gebruik te maken van een of meer biomarkers, zoals hippocampusatrofie op de MRI-scan, een typisch profiel van bèta-amyloïd en tauproteïne in de liquor of een positieve PIB-PET-scan, zal vermoedelijk in de nabije toekomst de ziekte van Alzheimer nog vóór het dementiestadiumkunnen worden vastgesteld. Validiteit en bruikbaarheid van deze biomarkers moeten echter nog nader worden vastgesteld.
Er is geen andere leeftijdsgeassocieerde ziekte die zo diffuus alle bronnen van welbevinden aantast, van lichamelijke tot psychologische en sociale. Daarom zijn zorgpaden voor dementie ook in de context van multimorbiditeit zinvolle ontwikkelingen.
