6. Wondgenezing

Samenvatting

Het proces van wondgenezing bestaat uit contractie, bindweefsel- en epitheelvorming, en kan worden beïnvloed door intrinsieke en extrinsieke factoren. Het uitgangspunt is om primaire wondgenezing na te streven en te voorkomen dat een acute wond overgaat in een chronische wond. Voor een goede wondbehandeling zijn het voorkomen van infectie en uitdroging, het reinigen van de wond en het sluiten van de ‘schone’ wond essentieel. Indien er sprake is van vertraagde wondgenezing, is behandelen van onderliggende pathologie essentieel. Wondgenezing kan in drie fases worden verdeeld. In de inflammatiefase komen de ontstekingscellen in de wond, die bacteriën en dood weefsel opruimen. In de proliferatiefase gaan endotheelcellen, keratinocyten en fibroblasten zich delen en differentiëren, waardoor granulatie (vaatnieuwvorming, collageensynthese) en epitheelvorming plaatsvinden. Ten slotte zullen in de remodelleringsfase de collageenfibrillen zich organiseren en sterker worden, waarbij er sprake is van een continue opbouw en afbraak. De matrix waarin het collageen ligt, wordt rijper en samen met het collageen wordt de wond daardoor sterker.
