Werken met de jeugdige met anorexia nervosa.

Aspecten van een psychiatrisch-psychotherapeutisch werkmodel

In de leerboeken staan weinig handvatten vermeld om vorm te geven aan een behandelingscontext voor een relatie met de adolescent met anorexia nervosa en diens gezin. In dit artikel wordt zowel vanuit de praktijk als vanuit de literatuur besproken hoe een behandelingscontext gecreëerd kan worden en hoe daarbinnen gezocht kan worden naar een therapeutische relatie in geval van anorexia nervosa. Een van de bekende hindernissen is dat adolescenten met anorexia nervosa zelden zelf om hulp vragen, oftewel: er is bij aanvang (nog) geen veranderingsmotivatie. Geschetst wordt hoe zakelijk, gericht op eten en gewicht, een behandelingsmodel neergezet wordt, resulterend in een behandelingsovereenkomst. Centraal staat de constatering van Vandereycken dat flexibiliteit vereist is en dat de therapeut dient te accepteren dat dit een proces van vallen en opstaan is. Separaat wordt kort ingegaan op de centrale plaats die angst in de eetstoornis inneemt en wordt stilgestaan bij het effect van de ontwikkelingsleeftijd op de vormgeving van de werkrelatie.
