Samenvatting

Men spreekt van icterus neonatorum prolongatus als een pasgeborene geel blijft, of gaat zien, na de tweede levensweek. Icterus neonatorum ontstaat als de serumbilirubineconcentratie hoger wordt dan 110-140 micromol/l, waarbij een gele verkleuring van huid en sclerae optreedt. Men spreekt van fysiologische icterus als deze rond de 2e-3e levensdag ontstaat, op de 7e levensdag verdwijnt en er sprake is van ongeconjugeerd bilirubinegehalte, niet hoger dan 210 micromol/l. Icterus neonatorum ontstaan vóór de 2e levensdag of na de 7e levensdag en gepaard gaand met een ongeconjugeerd bilirubine boven 210 micromol/l moet beschouwd worden als afwijkend. Een geconjugeerde hyperbilirubinemie duidt altijd op een aandoening. Hieronder volgen enkele richtlijnen die de huisarts kunnen helpen bij de besluitvorming ten aanzien van het beleid bij icterus neonatorum prolongatus.
