Voedingsadvies is effectief preventief

Een belangrijke barrière bij dieettherapie is dat, op individueel niveau, effecten op risicofactoren klein zijn in vergelijking met medicijnen. Kijken we echter naar de uitkomsten waar het werkelijk om gaat, namelijk het voorkomen van hart- en vaatziekten en voortijdige sterfte, dan blijken de effecten van dieettherapie en van statines vergelijkbaar te zijn. Dit geldt zowel voor primaire als voor secundaire preventie. Om bij secundaire preventie een coronair event of een voortijdig sterfgeval te voorkomen, is bij dieettherapie de ‘number needed to treat’ (NNT) respectievelijk ongeveer 92 en 125 (tot maximaal 500) per jaar, en bij statinetherapie 233 en 250. De resultaten van dieettherapie zijn gebaseerd op oude onderzoeken. Bij primaire preventie zijn wel positieve effecten aangetoond op hart- en vaatziekten, maar niet op sterfte. Dit wordt gedeeltelijk verklaard door methodologische problemen bij dergelijk wetenschappelijk onderzoek.
Huisartsen moeten zich ervan bewust zijn dat een dieetadvies naast effecten op traditionele risicofactoren, ook effecten heeft op andere processen, zoals ontstekingsmechanismen, endotheelactivatie en hartritmestoornissen. Deze effecten leiden gezamenlijk tot preventie van hart- en vaatziekten en een lagere voortijdige sterfte. Daarom is het onjuist de gezondheidswinst van een voedingsadvies te beoordelen binnen de kaders van een NHG-Standaard en te vergelijken met speciaal voor die risicofactor ontwikkelde medicatie.
