Vliegen en veneuze trombose

Samenvatting

De afgelopen 10 jaar is veel onderzoek gedaan naar een mogelijk verband tussen het maken van een lange vliegreis en het ontstaan van veneuze trombose. Epidemiologische studies laten zien dat het risico op trombose ongeveer driemaal is verhoogd na een reis van meer dan 4 uur. Absoluut gezien komt dit overeen met een risico van 1 op 4500 passagiers. Dit risico is duidelijk hoger voor mensen met factor V Leiden, voor vrouwen die de pil gebruiken en voor lange en kleine mensen. Bovendien neemt de kans op trombose toe met de duur van de vlucht en met het aantal keer dat men in korte tijd heeft gevlogen. Uit pathofysiologisch onderzoek blijkt dat stollingsactivatie optreedt na een vlucht van 8 uur. Naast een effect van immobilisatie is dit mogelijk toe te schrijven aan specifieke omstandigheden tijdens het vliegen, zoals de verlaagde zuurstofspanning die aanwezig is in de cabine. Er is nog onvoldoende onderzoek beschikbaar om een uitspraak te kunnen doen over het nut van het dragen van elastische kousen. Acetylsalicylzuur is niet voldoende effectief ter voorkoming van veneuze trombose en bovendien niet zonder risico’s. In afwachting van verder onderzoek is profylaxe met laagmoleculairgewichtheparine alleen te overwegen wanneer patiënten met een hoog tromboserisico, zoals mensen met een trombose in de voorgeschiedenis, een lange vlucht willen maken.
