Virale hepatitis: hoe zat het ook alweer?

Samenvatting

Virale hepatitis is een leverontsteking die wordt veroorzaakt door ‘hepatotrope virussen’, zoals hepatitis A tot en met E, die hoofdzakelijk levercellen infecteren. Het hepatitis A‑ en E‑virus (HAV en HEV) hebben een feco-orale transmissieroute en veroorzaken met name een acute hepatitis die veelal self-limiting is. HAV-infectie is meestal een importziekte (voornamelijk uit Marokko), maar HEV-infectie kan men ook oplopen door besmet vlees uit Nederland en komt steeds vaker voor. Een infectie met het hepatitis B‑ en C‑virus (HBV en HCV) kan zowel acuut als chronisch verlopen. De transmissie verloopt voor beide via bloed-bloedcontact, maar bij HBV ook via seksueel contact en perinataal. Een HBV-infectie wordt bij jonge kinderen meestal chronisch, maar volwassenen klaren HBV bijna altijd. HCV wordt in driekwart van de gevallen chronisch en voor beide virussen geldt dat chronisch geïnfecteerden vaak asymptomatisch zijn, maar wel risico lopen op het ontwikkelen van cirrose en hepatocellulair carcinoom op de lange termijn (20–30 jaar). Hepatitis D-infectie komt in acute en chronische vorm alleen voor bij patiënten die drager zijn van het hepatitis B‑virus, maar wordt in Nederland bijna niet gezien.
