7. Vergrote lymfeklieren

Samenvatting

Vergrote lymfeklieren zijn geen zeldzame klacht. Vaak is de verklaring een (banale) infectie of wond in het drainagegebied van de lymfeklier, zelden een tumor. Bij kinderen is een voorafgaande bovensteluchtweginfectie veelal de oorzaak van voelbare lymfeklieren, vooral in de hals. Wees vanwege de sterk verhoogde kans op een maligniteit alert bij een supraclaviculaire klier en bij een solitaire klier in de hals van een volwassene. Naast de lokalisatie kunnen de grootte, de consistentie en de gevoeligheid van de klier en de leeftijd van de patiënt een indicatie geven over de diagnose. Bij kinderen met een lymfeklier < 2 cm en aanwezigheid van een bovensteluchtweginfectie is een afwachtend beleid gerechtvaardigd. Bij mensen ouder dan 40 jaar is het meest verdacht: grootte > 1 cm, vaste consistentie van de klier, het ontbreken van pijn, supraclaviculaire of laagcervicale lokalisatie en de aanwezigheid van gegeneraliseerde jeuk. Aanvullend laboratoriumonderzoek kan helpen bij het vaststellen van een infectieuze oorzaak, maar kan niet differentiëren tussen een kwaadaardige aandoening en een infectie.
