46. Vergeetachtigheid (geheugenklachten)

Samenvatting beleid

Onderzoek is gericht op objectiveren van klachten en in kaart brengen van mogelijk beperkingen in het dagelijks functioneren. Uitsluiten van secundaire oorzaken kan grotendeels door middel van anamnese en lichamelijk onderzoek, op indicatie aangevuld met een beperkt bloedonderzoek. Verwijzing is alleen zinvol als er focale neurologische uitvalverschijnselen worden gevonden, of als er een sterke verdenking bestaat op dementie. Algemene maatregelen: begin met uitleg en geruststelling, eventueel door verwijzing naar www.​thuisarts.​nl/​vergeetachtighei​d/​ik-ben-vergeetachtig. Goede patiënteducatie is het best bewezen hulpmiddel om subjectieve geheugenklachten te verminderen. Adviezen: cognitieve geheugentraining is bewezen effectief in het verbeteren van het geheugen. Er zijn tegenwoordig tal van apps beschikbaar op de smartphone om mee te oefenen. Ook lichte fysieke inspanning werkt positief op alle vlakken van cognitie. Omdat er nauwelijks contra-indicaties zijn voor deze adviezen kunnen ze universeel gegeven worden aan patiënten met geheugenklachten. Er valt weinig effect te verwachten van het geven van voedingssupplementen. Als er geen aangetoonde deficiënties zijn, zijn deze niet aan te raden. Ook oosterse kruiden zoals ginkgo biloba en ginseng zijn niet effectief. Deze middelen hebben echter weinig bijwerkingen en hoeven niet actief afgeraden te worden. Medicamenteuze behandelingen, onder meer cholinesteraseremmers, zijn aangetoond niet zinvol. Vanwege de kans op bijwerkingen worden deze middelen afgeraden.
