Ventrale stabilisatie van thoracolumbale wervelfracturen

Samenvatting

Dorsale stabilisatie van thoracolumbale fracturen met behulp van pedikelschroeven is inmiddels een vertrouwde techniek met relatief weinig perioperatieve complicaties. Op deze wijze kan meestal worden volstaan met de fixatie van slechts een beperkt aantal bewegingssegmenten, meestal twee: één craniaal en één caudaal van de gefractureerde wervel. De mogelijkheden om daarnaast extra ventrale ondersteuning aan te brengen via deze benadering zijn echter beperkt. Indien het dragend vermogen als gevolg van het letsel te sterk is aangetast, is deze ondersteuning wel gewenst. De door Daniaux beschreven methode van transpediculaire spongiosaplastiek om hierin te voorzien leek aanvankelijk veelbelovend, maar bleek in de praktijk niet werkzaam. Het ontbreken van adequate ventrale ondersteuning leidt niet zelden tot het buigen, breken of loslaten van pedikelschroeven met repositieverlies als gevolg. Ook een posterieure spondylodese kan dit niet voorkomen. Als na verloop van tijd de fixateur interne wordt verwijderd, neemt repositieverlies meestal nog verder toe, waardoor niet zelden de oorspronkelijke posttraumatische stand van de wervelkolom terugkeert. Een ventrale stabilisatie van instabiele thoracolumbale wervelfracturen leidt tot minder repositieverlies en een grotere kans op een geslaagde spondylodese dan uitsluitend dorsaal uitgevoerde ingrepen. Door de invoering van thoracoscopische technieken kunnen deze procedures worden uitgevoerd met minder perioperatieve complicaties en minder postoperatieve pijnklachten. De toepassing van distraheerbare ‘cages’ hierbij heeft een aantal voordelen en, voor zover tot nu toe bekend, geen ernstige nadelen.
