2. Veiligheid en kwaliteit

Samenvatting

In dit hoofdstuk wordt aangegeven welke maatregelen er door onder andere de Nederlandse Vereniging voor Heelkunde (NVvH) worden genomen om de kwaliteit van zorg en patiëntveiligheid te optimaliseren. De Inspectie voor de Gezondheidszorg en Jeugd en de Nederlandse Zorgautoriteit (NZa) hebben hier een centrale rol bij. De IGJ houdt toezicht op de gehele zorg sector met als doel het waarborgen van de kwaliteit en veiligheid en het stimuleren van leren en verbeteren hiervan. Onmisbaar zijn de registraties van complicaties, incidenten, calamiteiten en fouten in de zorg. Vanaf 2010 heeft de NVvH een zogenoemd geïntegreerd kwaliteitsbeleid. De kwaliteitsnormen, opgesteld door de medisch beroepsgroepen zelf, hebben tot een aanzienlijke concentratie van laagvolume hoogcomplexe chirurgische zorg geleid in centers of excellence. Naast het beschrijven van de randvoorwaarden voor kwalitatief goede zorg, wordt er ook gemeten wat de kwaliteit van de geleverde zorg op ziekenhuisniveau is, in de vorm van landelijke ‘clinical audits’. Communicatie is een essentieel fundament achter veiligheid en effectiviteit in de gezondheidszorg. Professionele feedback op teamwork en teamgedrag hebben hun meerwaarde aangetoond, en ook de invoering van het elektronisch patiëntendossier (EPD) kan ondanks zijn tekortkomingen een bijdrage leveren. Teamtraining verbetert OK-prestaties. De effecten van stress en vermoeidheid op patiëntveiligheid zijn groot. Er hoort binnen de vakgroep chirurgie aandacht te zijn voor belasting en belastbaarheid. Transparantie over kwaliteit en veiligheid van zorg gaat in de komende jaren de mogelijkheid geven tot verbeteren op lokaal en nationaal niveau. Kortom, alle beschikbare middelen worden benut om de gezondheidszorg om te vormen tot een geoliede lerende organisatie.
