12. Vallen bij ouderen

Samenvatting

Vallen komt frequent voor en kent zowel lichamelijke als psychische gevolgen. De huisarts is slechts van 20–30 % van alle valincidenten op de hoogte. De belangrijkste valrisicofactoren zijn een doorgemaakte val in het afgelopen jaar (valangst) en de aanwezigheid van een mobiliteitsstoornis (loopstoornis). Goede kennis van de valrisicofactoren en een systematische aanpak van de diagnostiek zijn nodig voor het opstellen van een multifactorieel behandelplan. De diagnostiek bestaat uit een uitgebreide valanamnese en een uitvoerig mobiliteitsonderzoek, gecombineerd met een algemeen lichamelijk onderzoek. Bij de diagnostiek zijn naast de huisarts meestal meerdere disciplines zoals fysiotherapie en ergotherapie betrokken. Gezien de multifactoriële etiologie, complexiteit, en de beperkte betekenis van aanvullende diagnostiek kan het wenselijk zijn een patiënt met recidiverend ‘onverklaard’ vallen te verwijzen naar een klinisch geriater (valpoli).
