Uitbreiding van het model van traumatische stress met het perspectief van sociaal-ecologische gezinssystemen

Samenvatting

De afgelopen decennia is er onder systemische onderzoekers en clinici steeds meer interesse gekomen in problemen die gerelateerd zijn aan psychische traumatische stress. Maar onderzoek naar traumatische stress en de vele gevolgen ervan is voornamelijk gericht op intrapersoonlijke en fysiologische processen. Intrapersoonlijke modellen van traumatische stress hebben zeker nut, maar toch heeft deze benadering een aantal beperkingen. Er zijn nieuwe aanwijzingen dat proximale contexten – zoals het gezin – en distale sociaal-ecologische contexten – zoals politieke en omgevingssystemen – een groot effect kunnen hebben op de risico’s, verschijningsvormen en het herstel van traumatische stress. In dit artikel geven we een overzicht en samenvatting van het onderzoek naar traumatische stress en integreren we de bevindingen van intrapersoonlijke en systeemgerichte traumatische stress-modellen. We introduceren hier wat wij het dynamische sociaal-ecologische model van traumatische stress hebben genoemd, een heuristisch model voor de conceptualisatie van de interacties tussen verschillende vormen van traumatische stress. Dit model bevat drie dimensies van traumatische stress – typologie, ernst en tijdsverloop – waarmee duidelijk wordt op welke manier de risico’s, verschijningsvormen en het herstel van traumatische stress afhankelijk zijn van de systemen waarin mensen leven. We sluiten af met een bespreking van het onderzoek en de klinische implicaties van dit conceptuele model.
