21. Tumoren van dunne en dikke darm

Samenvatting

Dunnedarmtumoren zijn zeldzaam. De symptomatologie is weinig specifiek. Naast adenocarcinomen komen carcinoïden, sarcomen en lymfomen in de dunne darm voor. Bij mensen met een bekende maligniteit zijn metastasen in de dunne darm de meest frequent voorkomende tumoren. De primaire behandeling is chirurgisch; systemische therapie als primaire, adjuvante of palliatieve behandeling wordt voor bepaalde indicaties toegepast. Dikkedarmkanker, oftewel colorectaal carcinoom, komt veel voor. Naast omgevingsfactoren speelt genetische aanleg een belangrijke rol in de ontstaanswijze. Screening op occult bloedverlies wordt tussen 2014 en 2019 in Nederland geïmplementeerd. Belangrijkste symptomen zijn obstructie van het darmlumen en chronisch bloedverlies. Na het stellen van de diagnose door middel van een colonoscopie met biopten is stadiëring van de lokale tumoruitbreiding en het aantonen van afstandsmetastasen noodzakelijk. De primaire behandeling is chirurgisch. Voor het coloncarcinoom betekent dit een verwijdering van het aangedane darmdeel met de bijbehorende lymfeklieren. Het rectumcarcinoom wordt behandeld door middel van een totale mesorectale excisie (TME). De circumferentiële marge die hierbij wordt verkregen is goed te voorspellen met preoperatieve MRI en is een van de belangrijkste prognostische factoren. Bij een resectabel rectumcarcinoom wordt alleen bij een relatief hoog risico op lokaal recidief een korte voorbestraling gegeven, omdat dan de voordelen opwegen tegen de nadelen. Bij meer uitgebreide tumoren wordt een lange voorbestraling (vijf weken) in combinatie met chemotherapie gegeven, gevolgd door een wachtperiode om de tumor kleiner te laten worden. Daarna wordt de patiënt geopereerd. Bij ingroei van een colorectaal carcinoom in omliggende organen moet een en bloc, oftewel multiviscerale, resectie worden uitgevoerd. Aanvullende chemotherapie is alleen standaard na een colonresectie met lymfekliermetastasen (stadium III) of bij slechte prognostische factoren van de primaire tumor. Follow-up is in het bijzonder gericht op het opsporen van resectabele levermetastasen en metachrone tumoren, die beide opnieuw met curatieve intentie geopereerd kunnen worden. Bij uitgezaaide ziekte is palliatieve systemische therapie levensverlengend met een goede kwaliteit van leven, en kan bij een minderheid van patiënten met curatieve intentie worden gegeven om een secundaire resectie van metastasen mogelijk te maken.
