3. Trochanter major pijnsyndroom

Samenvatting

Laterale heuppijn komt bij ouderen veel voor en wordt vaak gediagnosticeerd als een bursitis trochanterica. Niet zelden bestaat de behandeling uit een corticosteroïdinjectie in de bursa. Een geïsoleerde bursitis komt echter zelden voor. Veel vaker is sprake van een peesprobleem rond de trochanter major al of niet in combinatie met een bursitis. De algemene term hiervoor is het trochanter major pijnsyndroom. Dit hoofdstuk beschrijft onder meer het ontstaansmechanisme, de bevindingen bij het klinisch onderzoek, de anatomie rond de trochanter major, de conservatieve therapie en enkele operatieve mogelijkheden.

De term trochanter major pijnsyndroom is gebaseerd op de terminologie in de Engelse literatuur: greater trochanteric pain syndrome. Deze term wordt hier gebruikt omdat er veel verwarring bestaat over de etiologie van lokale laterale heuppijn. Meestal wordt ten onrechte de diagnose bursitis trochanterica gesteld bij patiënten met dit type klachten.

Een diagnose stellen door andere mogelijkheden uit te sluiten.

In het geval van chronische achillespeesklachten wordt zwelling van de pees gewoonlijk wel opgemerkt doordat deze pees zich oppervlakkig onder de huid bevindt. Zwelling wordt bij tendinose veroorzaakt door een toename van de hoeveelheid matrix tussen de collagene vezels binnen de pees.
