In dit artikel bespreken we een aantal veelvoorkomende vraagstukken omtrent de diagnostiek en behandeling van tinnitus, met als uitgangspunt de recente promotie van de eerste auteur. We hopen hiermee huisartsen handvatten aan te reiken bij het nemen van de beslissing om een patiënt met tinnitus al dan niet door te verwijzen naar de kno-arts of het audiologisch centrum.
Tinnitus (‘oorsuizen’) definieert men als een auditieve fantoomperceptie van betekenisloze geluiden. Het is een veelvoorkomende aandoening (prevalentie 7-19%), waarvan de meeste patiënten lichte hinder ervaren. Een klein deel heeft echter forse klachten met een sterke vermindering van de kwaliteit van leven. De oorzaak van tinnitus is meestal gehoorverlies. In de meeste gevallen is er sprake van leeftijdgebonden (presbyacusis), lawaaigebonden of erfelijk gehoorverlies. De onderzoeken die in het proefschrift zijn opgenomen laten onder andere zien dat de ernst van de tinnituslast samenhangt met verschillende tinnituskarakteristieken en gezondheidsgerelateerde factoren, dat het niet zinvol is om bij alle tinnituspatiënten met persisterende (langer dan drie maanden bestaande) klachten een MRI-scan te laten verrichten en dat er geen behandeling bestaat die tot genezing leidt.
Wij adviseren om bij iedere tinnituspatiënt een screeningsaudiogram te verrichten. Op basis hiervan kunnen huisartsen beoordelen of ze de patiënt naar de kno-arts of het audiologisch centrum moeten doorverwijzen. Dat laatste moeten huisartsen overwegen bij patiënten met unilaterale tinnitus en/of ernstige tinnitusklachten nadat ze deze hebben geïnformeerd over de indicatie en effectiviteit van mogelijke behandelopties. We adviseren om de tinnituszorg stapsgewijs op te zetten.
