Samenvattting

Maligniteiten in de thoraxholte komen in Nederland veel voor. De meest voorkomende maligniteit in de thorax is het niet-kleincellig longcarcinoom. Zowel het niet-kleincellig longcarcinoom als het veel minder voorkomend kleincellig longcarcinoom kent een sterke relatie met roken. Zonder hen te stigmatiseren is het van belang dat zorgverleners patiënten ondersteunen bij het stoppen met roken. Veel patiënten hebben bij diagnose longkanker al een uitgezaaide ziekte, waardoor veel behandelingen noodgedwongen een palliatieve intentie hebben. Daarnaast hebben patiënten vaak veel klachten en symptomen door de uitgebreidheid van de ziekte. Dit maakt multidisciplinaire samenwerking bij deze aandoening van wezenlijk belang. Toch is de prognose van longkankerpatiënten de afgelopen jaren verbeterd. Enerzijds door verbeterde diagnostische technieken, anderzijds door de invoering van immuuntherapie en doelgerichte therapie. De behandeling van het maligne pleurale mesothelioom is de afgelopen jaren niet echt verbeterd. Veel patiënten presenteren zich met een stadium III- of IV-ziekte waar curatie niet meer tot de mogelijkheden behoort. Chemotherapie als systeembehandeling en radiotherapie voor lokale controle en pijnbestrijding zijn de belangrijkste behandelmogelijkheden. Het thymoom is een zeldzame tumor van de thymus. In Nederland worden per jaar ongeveer 60–70 mensen met deze diagnose geconfronteerd. Gezien de zeldzaamheid van deze tumor zijn er in Nederland initiatieven om voor deze maligniteit een landelijk multidisciplinair overleg (MDO) op te zetten.
