24. Thoracale aorta

Samenvatting

De thoracale aorta is het gedeelte van de grote lichaamsslagader dat in de borstholte gelegen is. We onderscheiden hierbij de aorta ascendens, de arcus aortae en de aorta descendens. Aandoeningen van de thoracale aorta zijn degeneratief, aangeboren of posttraumatisch. Er kan sprake zijn van dilaterend vaatlijden, stenoserend vaatlijden of het acuut aortasyndroom. Hieronder vallen: dissectie, penetrerend aorta ulcus en intramuraal hematoom. De thoracale aorta kan ook een gehele of partiële transectie doormaken in een (stomp) hoog energetisch trauma. Behandeling van een aandoening van de thoracale aorta kan in de acute setting plaatsvinden of preventief zijn om een ruptuur te voorkomen. Bij een preventieve behandeling zal het operatierisico worden afgewogen tegen het ruptuurrisico. Bij de behandeling kennen we een klassieke open operatie of een endovasculaire behandeling met endoprothese. Indien mogelijk wordt vaak gekozen voor een endovasculaire behandeling in verband met het minder invasieve karakter van deze behandeling.
