13. Tandheelkundige gevolgen van twee vormen van osteomalacie

Samenvatting

Osteomalacie is een ziektebeeld waarbij bot zijn hardheid verliest, doordat nieuwgevormd bot verminderde mineralisatie vertoont. Voor de tandarts zijn als oorzaken van osteomalacie met name de hypofosfatasie en hypofosfatemische rachitis interessant, vanwege de ongebruikelijke symptomatologie. Door een mineralisatiestoornis van het cement kan er bij hypofosfatasie vroegtijdig verlies van vitale melkelementen met volledige wortel optreden. Bij (X-gebonden) hypofosfatemische rachitis ontstaan er multipele spontane dentogene abcessen. De aandoening ontstaat door een verstoring van de mineralisatie en toename van de porositeit van dentine met scheurvorming tot gevolg. Hierdoor is het element verminderd beschermd tegen bacteriële invasie en kan spontaan dentogene abcesvorming optreden. In dit hoofdstuk worden beide ziektebeelden besproken inclusief etiologie, pathogenese en casuïstiek.
