Taaltests voor, tijdens en na wakkere hersenchirurgie: de rol van DuLIP

Samenvatting

Neurochirurgie bij patiënten met hersentumoren in eloquente zones beoogt een maximale resectie van de tumor met een minimaal risico op postoperatieve functionele uitval, zoals taalstoornissen (afasie). Inzicht in de functie en de exacte lokalisatie van corticosubcorticale gebieden en hun verbindingen is daarbij van cruciaal belang. Momenteel geldt de meer invasieve techniek, de directe elektrische stimulatie (DES) als gouden standaard. Om essentiële gebieden voor taal te bepalen, worden via DES een aantal corticale en subcorticale structuren rechtstreeks gestimuleerd met elektroden, terwijl de patiënt wakker is en diverse taaltaken uitvoert. Er zijn echter geen richtlijnen beschikbaar, noch een wetenschappelijk onderbouwde taaltestbatterij, om taalfuncties betrouwbaar te bewaken tijdens een wakkere operatie. In dit artikel wordt een overzicht geboden van de pre-, intra- en postoperatieve taaltests die in Vlaanderen en Nederland gebruikt worden. Daarnaast wordt de ontwikkeling van de Nederlandse taaltestbatterij (Dutch Linguistic Intraoperative Protocol) beschreven. Deze batterij bestaat uit fonologische, semantische en syntactische taalproductie- en perceptietaken die intraoperatief kunnen worden gebruikt tijdens corticosubcorticale stimulatie. Daarnaast worden in dit artikel anatomisch-klinische modellen gepresenteerd die als basis dienen voor de selectie van DuLIP-taken bij tumorpatiënten. Tot slot wordt het gebruik van dit protocol geïllustreerd met een casusbeschrijving.
