Taalstoornissen bij dementie deel 2: functionele neuroanatomie en leidraad voor de praktijk

Samenvatting

De bekendste vormen van afasie berusten op acuut ontstane gelokaliseerde afwijkingen in de dominante hemisfeer van de hersenen, meestal na een cerebrovasculair accident. Afasie kan ook deel zijn van de cognitieve stoornissen bij dementie. Bij primair progressieve afasie (PPA) is afasie het hoofdsymptoom, dat jarenlang als geïsoleerde stoornis voorafgaat aan, vooral frontotemporale, dementie. Onderzoek van PPA-varianten heeft ons veel geleerd over de rol die spraak-taalstoornissen spelen in ons denken en communiceren en de daaraan ten grondslag liggende veranderingen in complexe taalnetwerken. Deze inzichten zijn ook van belang voor de beoordeling van meer algemene beperkingen van de taal en communicatie bij diverse vormen van dementie. Kennis hiervan biedt nieuwe aanknopingspunten voor behandeling en begeleiding van mensen met dementie.
