19. Suïcidaliteit en autisme

Samenvatting

Er is steeds meer belangstelling voor autisme, of beter gezegd autismespectrumstoornissen (ASS), vooral als het gaat om ASS bij volwassenen, want daar was tot twintig jaar geleden nog weinig over bekend. Er is ook nog maar weinig bekend over suïcidaliteit bij mensen met ASS. Met het besef dat ASS ook voorkomt bij volwassenen en dat het spectrum veel breder is dan tot dusver werd gedacht, is er een verschuiving in de diagnostiek gekomen. Vooral bij chronische patiënten die niet goed reageren op therapie en voor wie werd uitgegaan van diagnoses als schizofrenie, borderlinepersoonlijkheidsstoornis of bipolaire stoornis, wordt nu regelmatig na herdiagnostiek de diagnose ASS gesteld. Daarnaast wordt de diagnose ASS eerder overwogen als mensen een burn-out krijgen en nauwelijks lijken te herstellen na ogenschijnlijk goed gefunctioneerd te hebben. Soms is de diagnose ASS (nog) helemaal niet gesteld, maar is er wel sprake van suïcidaliteit. Uit de bespreking van de literatuur zal blijken dat dan de eerste presentatie van ASS juist die suïcidaliteit is. Om suïcidaliteit bij ASS beter te begrijpen is kennis over ASS noodzakelijk. In dit hoofdstuk zal daarom ruim aandacht worden besteed aan het klinisch beeld van ASS. Naast bestudering van de literatuur is de praktijk de beste leerschool. Die praktijk wordt aan de hand van twee vignetten verkend. Tot slot zullen aanbevelingen en aandachtspunten worden geformuleerd voor het omgaan met suïcidaliteit bij ASS in de klinische praktijk, vanuit meerdere perspectieven.
