16. Strategieën voor bevordering van patiëntveiligheid

Samenvatting

Patiëntveiligheid is van oudsher een belangrijk aspect van kwaliteit van zorg. Een adverse event is een onbedoelde uitkomst, ontstaan door het handelen van een zorgverlener en/of door het zorgsysteem met schade voor de patiënt: tijdelijke of permanente beperking, verlenging of verzwaring van de behandeling dan wel overlijden van de patiënt. De kans op adverse events is hoger bij bepaalde patiënten, interventies, geneesmiddelen en omstandigheden. Bij het herkennen en leren hanteren van onveilige situaties zijn het opsporen, vastleggen, melden, meten en analyseren van incidenten cruciale stappen. Meestal wordt veel tijd en energie besteed aan het meten van incidenten en het analyseren van risicovolle processen en relatief weinig aan het opzetten en evalueren van verbeterinterventies. Bij het analyseren van de oorzaken van patiëntveiligheid gaat veel aandacht uit naar systemen. Werkomstandigheden (apparatuurontwerp, aantal zorgverleners, werkbelasting, logistiek van zorgprocessen, stresserende factoren) moeten zo aangepast worden dat fouten niet of minder zullen optreden. Specifieke interventies voor verbetering van patiëntveiligheid zijn onder meer: aandacht aan veiligheid in scholing, versterking van teams voor patiëntenzorg, inzet van informatietechnologie, verbetering van veiligheidscultuur, betrekken van patiënten, zorgpaden en checklists, verbetering apparatuurontwerp, infectiepreventiebundels en valpreventie. Het bewijs voor de effectiviteit van veiligheidsbevorderende interventies is over het algemeen beperkt.
