Onvrijwillig verlies van ontlasting (fecale incontinentie, FI) is een onderbelicht probleem, dat groot effect heeft op de kwaliteit van leven. Onder ouderen in Nederland loopt de prevalentie op tot 16%. De etiologie van FI is heel divers. Waar mogelijk moet de oorzaak worden opgespoord en behandeld; inflammatoire darmziekte en maligniteiten zijn soms de oorzaak van FI, en fecale impactie komt vaak in verpleeg- en verzorgingshuizen voor. Aanvullend onderzoek in de eerste lijn kan bestaan uit coloscopie of een CT-scan van het abdomen. Voor de behandeling van FI kan bekkenbodemtherapie van nut zijn, maar zeker ook psylliumvezels en diarreestoppers, zoals loperamide. Houd bij verwijzing naar de tweede lijn rekening met de vitaliteit van de patiënt. In de tweede lijn kan uitgebreide diagnostiek plaatsvinden (anale echografie, anorectale manometrie, dynamische MRI en bekkenbodem-emg) en zijn veel vormen van therapie mogelijk (onder andere lauwwaterspoeling, chirurgisch ingrijpen).
