Standaard lymfeklierdissectie bij blaascarcinoom: significante verschillen in het aantal gerapporteerde lymfeklieren

Samenvatting

Doelstelling:

In deze studie werd het aantal gerapporteerde lymfeklieren na histopathologische evaluatie van uitgebreide bilaterale pelviene lymfeklierdissecties (PLKD) vanwege blaaskanker vergeleken tussen 2 verschillende ziekenhuizen. Aanvullend werd de overleving van beide groepen patiënten geanalyseerd.

Materiaal en methoden:

Alle opeenvolgende patiënten, die een PLKD ondergingen in verband met blaaskanker in de periode van 1-1-2007 tot 31-12-2009 werden geïdentificeerd. De operaties werden in beide ziekenhuizen verricht door dezelfde 4 urologen op basis van een gestandaardiseerd PLKDtemplate. De preparaten werden in een minimum van 2 pakketten per zijde aangeleverd aan de afdeling Pathologie van het desbetreffende ziekenhuis. Beide pathologieafdelingen verwerkten de preparaten volgens hun eigen protocollen. Overlevingscijfers werden berekend met behulp van de Kaplan-Meiermethode.

Resultaten:

174 patiënten werden geïncludeerd (gemiddelde leeftijd: 62,7 jaar). In ziekenhuis A werden gemiddeld 16 lymfeklieren gevonden versus 28 klieren in ziekenhuis B (p < 0,001). Er was geen significant verschil in het aantal tumorpositieve lymfeklieren (p = 0,65). De lymfeklierdichtheid in ziekenhuis A was 9,3% versus 3,9% in ziekenhuis B (p = 0,056). De mediane follow-upduur was 3 jaar. De overall 2-jaarsoverleving, ziektespecifieke overleving en recidiefvrije overleving voor patiënten in ziekenhuis A was respectievelijk 61%, 64% en 54%, en voor patiënten in ziekenhuis B respectievelijk 58%, 58% en 53% (niet statistisch significant verschillend). Ook na correctie voor pT-stadium en neoadjuvante chemotherapie waren de overlevingskansen niet significant verschillend tussen ziekenhuis A en B.

Conclusie:

Ondanks anatomisch overeenkomstige dissecties door dezelfde ervaren urologen, toont deze studie een statistisch significant verschil in het aantal gerapporteerde lymfeklieren tussen 2 verschillende ziekenhuizen, na PLKD vanwege blaaskanker. Deze verschillen komen niet tot uiting in de overleving van de 2 groepen. Zolang de pathologische evaluatie niet gestandaardiseerd is, moet men voorzichtig zijn met het gebruik van het aantal gerapporteerde lymfeklieren als maatstaf voor de kwaliteit van chirurgie en lymfeklierdichtheid als prognostische factor.
