64. Sombere stemming

Samenvatting

Bij klachten van een sombere stemming is het van belang om een onderscheid te maken tussen enerzijds een somber gevoel als invoelbare reactie op een psychosociaal probleem of overspanning en anderzijds een pathologische somberheid die niet in relatie staat tot de uitlokkende factor qua ernst en duur van de klachten. Pathologische somberheid gaat samen met verschillende neurovegetatieve verschijnselen en leidt tot disfunctioneren op sociaal- en beroepsmatig vlak. De diagnose depressie wordt gesteld als aan specifieke criteria wordt voldaan. Een afzonderlijke categorie vormen de patiënten met een (meestal mildere) chronische vorm van somberheid; de persisterende depressieve stemmingsstoornis of dysthymie. Een (ernstige) depressie gaat incidenteel gepaard met psychotische kenmerken zoals wanen. Als er naast de depressie ook manische perioden voorkomen spreekt men van een bipolaire stemmingsstoornis. Ook een aantal somatische aandoeningen kunnen tot een sombere stemming of een depressieve stoornis leiden, waaronder schildklierfunctiestoornissen, een cerebrovasculair accident, de ziekte van Parkinson en beginnende dementie. Het belangrijkste diagnosticum van de arts is de anamnese en het psychiatrisch onderzoek. Hierbij moet men goed doorvragen, geen genoegen nemen met vage antwoorden en tijdens het gesprek letten op objectiveerbare afwijkende gedragingen van de patiënt. De arts gaat hierbij altijd met behulp van gerichte vragen na of er een risico op suïcide aanwezig is. De diagnostiek bij somberheid kan complex zijn als patiënten zich met een somatische klacht presenteren. Specifieke klinische vragenlijsten, zoals de 4DKL kunnen niet alleen de arts helpen bij het herkennen en diagnosticeren van een depressie, maar kunnen ook patiënten inzicht geven in de ernst van hun klachten en de resultaten van behandeling. Depressie, stress, angst en alcohol- of middelenmisbruik zijn vaak met elkaar verweven. Patiënten presenteren zich in de eerste lijn vaak met onvolledige en atypische beelden, waardoor de herkenning van stemmingsstoornissen een uitdaging kan zijn in de dagelijkse praktijk. Stemmingsstoornissen gaan dikwijls gepaard met negativisme: gevoelens van schuld en waardeloosheid en een gebrek aan concentratie. Dit kan met zich meebrengen dat deze patiënten terughoudend zijn met het vragen van hulp. Dit type problemen vergt daarom een actieve benadering van de arts, waarbij het herhaaldelijk beoordelen van de klachten van de patiënt soms noodzakelijk is om tot een diagnose te komen.
