Sociale cognitie bij frontotemporale dementie

Samenvatting

Hoewel bekend is dat beperkingen in sociale cognitie een kernsymptoom zijn bij patiënten met de gedragsvariant van frontotemporale dementie (bvFTD) was het tot op heden nog niet bekend of stoornissen in de sociale cognitie ook onderscheid kunnen maken tussen bvFTD en de brede differentiaaldiagnose in de klinische praktijk. Het doel van onze studie was om te onderzoeken of test voor de sociale cognitie bvFTD onderscheidt van andere neurodegeneratieve ziekten en psychiatrische stoornissen bij patiënten met een laat ontstaan frontaal syndroom. In deze longitudinale multicenter studie werd sociale cognitie bepaald door de Ekman 60 Faces-test en de Faux Pas-test te gebruiken bij patiënten met waarschijnlijke/definitieve bvFTD (n = 22), andere neurodegeneratieve ziekten (n = 24) en psychiatrische stoornissen (n = 33). De mediane duur van de symptomen was 2,8 jaar en patiënten werden prospectief gevolgd over een periode van twee jaar. Totaalscores op de Ekman 60 Faces-test waren significant lager bij bvFTD dan bij andere neurodegeneratieve ziekten en psychiatrische stoornissen. De Ekman 60 Faces-test verklaarde 91,2 % van de variantie van psychiatrische stoornissen en andere neurodegeneratieve ziekten versus bvFTD (χ = 11,02; df = 1; p = 0,001). Prestaties op de Faux Pas-test en de andere cognitieve domeinen verschilden niet tussen deze diagnosegroepen. In deze studie onderscheidde de Ekman 60 Faces-test bvFTD met succes van andere neurodegeneratieve ziekten en psychiatrische stoornissen. Deze studie biedt argumenten om de Ekman 60 Faces-test toe te voegen aan het neuropsychologisch onderzoek in de diagnostische procedure van bvFTD.
