Sociaaleconomische status en verschillende gezondheidsaspecten van ouderen

Achtergrond:

Onderzoek naar sociaaleconomische status (SES) en gezondheid bij ouderen in Nederland is relatief beperkt en er wordt merendeels gebruik gemaakt van opleiding als sociaaleconomische indicator.

Doel:

In dit artikel wordt verslag gedaan van onderzoek naar sociaaleconomische verschillen in gezondheid bij ouderen, met zowel opleiding als drie verschillende operationalisaties van inkomen als sociaaleconomische indicator.

Methode:

Gebruik is gemaakt van cross-sectionele surveygegevens over 2004-2006, waaraan fiscale inkomensdata zijn gekoppeld. De onderzochte gezondheidskenmerken betreffen zelfrapportages van medisch zorggebruik, fysieke en psychische gezondheidsstatus en beperkingen in Algemene Dagelijkse Levensverrichtingen (ADL). Met logistische regressie-analyse zijn deze kenmerken gerelateerd aan opleiding en (telkens) een operationalisatie van inkomen. In aanvullende blokken is gecorrigeerd voor demografische kenmerken en voor relevante gezondheidskenmerken.

Resultaten:

Besteedbaar inkomen, al dan niet over meerdere jaren, en opleiding maken weinig onderscheid in het raadplegen van huisarts, specialist en fysiotherapeut. Dit geldt ook voor langdurig laag inkomen, de derde operationalisatie van inkomen. Wel discrimineren de eerste twee inkomensmetingen in fysieke en psychische gezondheidsstatus en in beperkingen in dagelijkse levensverrichtingen. Het langdurig laag inkomen onderscheidt alleen in psychische gezondheid.

Conclusie:

De hier onderscheiden gezondheidskenmerken blijken gerelateerd aan inkomen ook na correctie voor relevante confounders. De discriminerende waarde van de inkomensspecificaties is bovendien groter dan die van opleiding. Inkomen vervult dan ook een spilfunctie in de beschrijving van sociaaleconomische gezondheidsverschillen bij ouderen.
