3. Shock

Inleiding

Shock wordt gedefinieerd als een inadequate orgaanperfusie en weefseloxygenatie met als uiteindelijk gevolg orgaandisfunctie. De ontwikkeling van shock is direct afhankelijk van de hoeveelheid geoxygeneerd bloed die aan de weefsels wordt aangeboden. Het hartminuutvolume, de hoeveelheid bloed die per minuut wordt weggepompt door het hart, is het product van de hartfrequentie en het slagvolume. Het slagvolume is afhankelijk van de preload, contractiliteit van het hart en de afterload. De preload wordt gevormd door de hoeveelheid veneus bloed die wordt aangeboden vanuit de vena cava. Deze wordt met name bepaald door de vullingsstatus van de patiënt en de druk in de thorax. De hoeveelheid veneus bloed die het hart vult, bepaalt de lengte van de hartspiervezels. De lengte van deze vezels is gerelateerd aan de contractiekracht van de hartspier, ofwel contractiliteit (wet van Starling). Afterload is de systemische perifere vaatweerstand.
