Screening op het downsyndroom en neuralebuisdefecten

Samenvatting

Tijdens de zwangerschap kunnen ouders in Nederland sinds kort kiezen voor betrouwbare bepaling van de kans op het downsyndroom rond de 11e week van de zwangerschap en voor screenen op neuralebuisdefecten rond de 20e week. Om de kans op het downsydroom te bepalen wordt informatie over de leeftijd van de zwangere gecombineerd met hormoonspiegels in het bloed van de moeder en het uiterlijk van de foetus. Als de kans op het downsyndroom groter blijkt dan 1 : 200 kunnen de ouders kiezen voor vervolgonderzoek in de vorm van een vlokkentest of vruchtwaterpunctie. Onderzoek naar neuralebuisdefecten vindt plaats tijdens het structureel echoscopisch onderzoek (SEO) rond de 20e week van de zwangerschap. Als het SEO bevindingen oplevert die mogelijk samenhangen met spina bifida of andere ernstige lichamelijke afwijkingen, kunnen de ouders kiezen voor gedetailleerd echo-onderzoek in een derdelijnscentrum.
Belangrijke aspecten bij deze nieuwe onderzoeken zijn de keuzevrijheid van de ouders en een uitgebreide ondersteunende organisatie die de kwaliteit van de voorlichting en de uitvoering bewaakt.
