51. Schouderklachten

Samenvatting

Schouderklachten komen veel voor in de huisartsenpraktijk. De klachten worden meestal veroorzaakt door structuren in de subacromiale ruimte en minder vaak door het schoudergewricht zelf of de structuren van de schoudergordel. Soms zijn ze een gevolg van ernstig trauma (fracturen, luxaties, rotatorcuffruptuur), aandoeningen buiten de schouderregio (pijn door longtoptumor of gerefereerde pijn door hartinfarct, longembolie of acute buikproblemen) of systemische reumatische aandoeningen. Op basis van het klinisch beeld zijn de niet-traumatische schouderklachten onder te verdelen in het subacromiaal pijnsyndroom (SAPS; pijn en/of bewegingsbeperking van het abductietraject), glenohumerale gewrichtsklachten (pijn en bewegingsbeperking bij zowel abductie als exorotatie die zich uit in bijvoorbeeld artrose of een frozen shoulder) en overige schouderklachten (o.a. klachten van de cervicale wervelkolom of het AC-/SC-gewricht). Beeldvormend onderzoek in de vorm van echografie, röntgenfoto of MRI kan van toegevoegde waarde zijn bij langdurige klachten, waarbij de gebruikelijke conservatieve behandelingen geen effect hebben gehad.
