12. Samenwerking en afstemming in de zorg voor kwetsbare ouderen (theorie)

Samenvatting

Het zorgstelsel heeft van oudsher een reactief karakter: er wordt pas actie ondernomen op het moment dat iemand klachten heeft. Daarnaast wordt binnen de zorg vaak nog in afzonderlijke domeinen (preventie, zorg, welzijn) gedacht. Het gevolg is dat zorg aan ouderen niet tijdig wordt aangeboden, het aanbod vaak versnipperd is en onvoldoende aansluit op de zorgvraag. De veronderstelling is dat kwetsbare ouderen baat hebben bij een meer proactief en integraal aanbod van zorg en ondersteuning. Mede vanuit deze gedachte is het Nederlandse zorgsysteem het laatste decennium sterk in beweging. Dit hoofdstuk beschrijft hoe de zorgomgeving van kwetsbare ouderen beter kan aansluiten bij hun zorg- en ondersteuningsbehoefte door een proactief en integraal aanbod, en de ontwikkeling van ziektespecifieke integrale benadering naar meer integrale benadering vanuit het belang van de populatie. Een aantal concrete aangrijpingspunten om zorg en ondersteuning van kwetsbare ouderen te verbeteren, wordt gegeven.
