Ruptuur van extensor pollicis longuspees secundair aan volair ingebrachte zelftappende hoekstabiele schroef

Samenvatting

Instabiele comminutieve distale radiusfracturen worden frequent behandeld met open repositie en interne fixatie met behulp van een plaatosteosynthese. Hierbij geeft een volaire ten opzichte van een dorsale plaatfixatie een kleiner risico op dorsale wekedelenschade. De incidentie van een zeldzame complicatie van de distale radiusfractuur, een ruptuur van de extensor pollicis longuspees, lijkt echter te zijn toegenomen bij het gebruik van volaire plaatosteosynthese. De mogelijke oorzaak hiervan is beschadiging van het derde extensorcompartiment veroorzaakt tijdens het boren of door protrusie van schroefpunten.

Onze casus betreft een 35-jarige man behandeld met volaire plaatosteosynthese wegens een distale radiusfractuur, die enkele maanden daarna een peesruptuur van de extensor pollicis longus kreeg.

Aan de hand van deze casus bespreken wij de etiologie en literatuur van extensor pollicis longuspeesrupturen bij distale radiusfracturen behandeld met een volaire plaatosteosynthese.
