Rugpijn is voor veel ouderen een belangrijk probleem. Een aanzienlijk deel van hen heeft langdurig rugpijn en ondervindt fysieke beperkingen. Die kunnen leiden tot inactiviteit, wat weer kan leiden tot meer rugpijn en meer fysieke beperkingen.
Aan rugklachten kan een ernstige onderliggende aandoening ten grondslag liggen, maar dit lijkt bij ouderen niet veel vaker het geval dan in andere leeftijdscategorieën. Dat geldt ook voor de diagnose ‘wervelfractuur’. Alarmsymptomen hebben weinig voorspellende waarde.
Onderzoek heeft tot nog toe weinig duidelijkheid opgeleverd over de variabelen die geassocieerd zijn met een ongunstige prognose. Het is dan ook nog niet goed mogelijk ze in de praktijk te gebruiken om het beloop te voorspellen of een gerichte behandeling aan te bieden.
In onze onderzoekspopulatie werden pijnstillers veel gebruikt, maar hun effectiviteit lijkt gering. Bij ouderen is bovendien het risico op bijwerkingen relatief groot. Het is daarom belangrijk de voor- en nadelen van pijnstillers – ook vrij verkrijgbare – met de patiënt te bespreken.
