Samenvatting

Voorgeschiedenis, anamnese en gericht lichamelijk onderzoek zijn belangrijke hulpmiddelen voor de arts bij de diagnostiek van een rood oog. Conjunctivitiden komen verreweg het meest voor. Iritis en scleritis zijn geassocieerd met systeemziekten. Een ernstig trauma van de oogkas kan leiden tot een orbitabodemfractuur met inklemming van oogspieren en een contusie of perforatie van de oogbol. Pijn, lichtschuwheid, verminderde visus en pupilafwijkingen zijn signalen die wijzen op een ernstige oogaandoening. Over de voorspellende waarde van deze symptomen werden echter geen publicaties gevonden. ’s Ochtends aan elkaar geplakte oogleden kunnen wijzen op een bacteriële conjunctivitis. Jeuk is het belangrijkste kenmerk van allergische conjunctivitis. Het dragen van contactlenzen is een risicofactor voor het ontstaan van cornea-ulcera. Pathologie van de cornea, zoals keratitis en het cornea-ulcus, gaat gepaard met pericorneale roodheid en een vermindering van het gezichtsvermogen. Kenmerkende bevindingen bij iridocyclitis zijn een eenzijdig nauwe of vervormde pupil, een pijnlijke consensuele lichtreactie en visusvermindering. Ernstige pijn, braken, een middelwijde, lichtstijve pupil en een palpatoir verhoogde oogboldruk wijzen op acuut glaucoom. Met behulp van spleetlamponderzoek kunnen kleine corpora aliena, lokale ontstekingsprocessen en afwijkingen in de structuren van het voorste oogsegment worden opgespoord, bijvoorbeeld een microperforatie. De waarde van het overig aanvullend onderzoek bij een acuut rood oog is beperkt.
