6. Richtlijnen als hulpmiddel bij de verbetering van de zorg

Samenvatting

Een richtlijn is een document met aanbevelingen ter ondersteuning van zorgprofessionals en zorggebruikers, gericht op het verbeteren van de kwaliteit van zorg en berustend op wetenschappelijk onderzoek aangevuld met expertise en ervaringen van zorgprofessionals en patiënten. Richtlijnen zijn bedoeld om zorgverleners te ondersteunen en kunnen bijdragen aan sturing en controle van de gezondheidszorg. Specifieke gebruiksdoelen zijn: samenvatting van wetenschappelijke kennis, vermindering van ongewenste variatie in zorgverlening, aanbevelingen voor goede zorg, onderbouwing van keuzen, basis van scholing en toetsing, basis voor samenwerkingsafspraken, basis voor agendasetting en prioritering. Beperkingen van richtlijnen zijn onder meer: gevaar van ‘kookboekgeneeskunde’, wekken van irreële verwachtingen, angst voor juridische consequenties, misbruik door autoriteiten en financiers, misbruik door beroepsgroepen en onzekerheid over kosteneffectiviteit. De ontwikkeling van een richtlijn dient een systematische, stapsgewijze aanpak te volgen, waarbij in elke stap aandacht wordt besteed aan verschillende aspecten van implementatie: onder meer relevantie, draagvlak, attractiviteit en toegankelijkheid. Er zijn instrumenten om de implementeerbaarheid van een richtlijn te beoordelen. AGREE (Appraisal Instrument for Guidelines, Research and Evaluation) is een instrument om de methode en de wetenschappelijke validiteit, maar ook de helderheid en toepasbaarheid van de richtlijn te beoordelen. Op het gebied van richtlijnontwikkeling wordt internationaal samengewerkt in het G-I-N (Guidelines International Network).
